Excessive use of food, drugs, sex, etc.

Online-Therapy

Availability

• Accessible worldwide.
• Providers are based in five countries: the U.S., Canada, the U.K., Australia, and New Zealand. This raises concerns, as therapists are typically licensed to practice only within the region where their license was issued.

• The platform is intended for individuals aged 18 and over who are not in immediate crisis (e.g., suicidal ideation, abuse, or domestic violence).

Insurance

Does not accept insurance.

Psychiatry (Medication)

No access to psychiatrists for prescribing and managing medication.

Nature of the Services

• Structured 8-session Cognitive Behavioral Therapy (CBT) program, usable with or without live therapy sessions.
• Each session includes worksheets for practicing therapeutic skills. These materials may feel too general or insufficiently tailored to specific concerns.
• The program and accompanying worksheets are the same for all users. While they may be helpful for a range of difficulties, some users may find them insufficiently adapted to their individual needs.
• All plans include unlimited therapist messaging (not live).
• Live 45-minute therapy sessions are available once per week with the Standard Plan and twice per week with the Premium Plan; there is no live session in the basic plan. Sessions may be conducted via video, voice, or text chat.
• All providers use a cognitive-behavioral approach. While this is the most evidence-based approach, it may not be the most appropriate for all users.

Choice of therapist

• You may choose a therapist from a list based on region, or be matched automatically during sign-up. Therapists can be changed if unsatisfied.

Progress tracking

No tool for monitoring progress.

• Standard – $288/month ($72/week): includes everything, plus one live 45-minute session per week.

Research

To the best of our knowledge, Headspace is the most studied mobile app for mental health and well-being, which is no small feat as there are over 10,000 such apps.

A systematic review of scientific literature identified 14 randomized controlled trials (the gold standard for intervention studies) that tested Headspace’s effectiveness. Thirteen studies were not funded by Headspace, reducing the risk of conflict of interest. The studies included different types of participants, such as beginners in meditation, college students, employees with elevated work stress, people with cancer, and individuals with signs of compulsive internet use. In almost all the studies, participants were instructed to use specific parts of the app for at least 10 minutes a day. The studies evaluated whether Headspace led to improvements in mindfulness (evaluated in 7 studies), well-being (8 studies), stress (5 studies), anxiety (4 studies), or depression (4 studies), for a total of 28 outcomes. Based on the review, 15 outcomes (53%) showed improvements, 3 outcomes (11%) were mixed (i.e., more than one measure was used to assess a single variable, like stress, but not all showed improvements), and 10 outcomes (36%) showed no improvement.

The authors of this meta-analysis stated that they had no conflicts of interest.

A meta-analysis identified 13 randomized control trials testing the efficacy of Headspace, among which 11 tested the effects on depressive symptoms, and 11 tested the effect on anxiety. When analyzing the data from all these studies together, they found small but significant effects on depressive symptoms and anxiety. None of the studies specifically targeted individuals with a diagnosed mental health disorder, although some recruited participants based on elevated symptoms of anxiety, worry, or stress.

The authors of this meta-analysis stated that they had no conflicts of interest.

Randomised controlled trials testing the efficacy of headspace continue being published. For example, one study conducted in California with 138 non-faculty university employees tested the effects of using Headspace daily for 10 minutes over an 8-week period. Participants were randomly assigned to either use Headspace daily for 10 minutes or to a control group. Compared to the control group, Headspace users experienced rapid and sustained improvements in the perceived ability to cope with stress, as well as reductions in stress and repetitive negative thinking (rumination and worry).

The authors of this article stated that they had no conflicts of interest.

Overall, there is strong scientific evidence that Headspace produces small but significant improvements in important aspects of mental health, such as anxiety and depressive symptoms. Many of the randomized controlled trials used active control groups, meaning participants in the control group received a low-intensity intervention. This makes it harder to detect differences between the Headspace group and the control group, since the latter also had some opportunities to improve. To maximize the chances of benefiting from the app, starting with the basic meditation courses and gradually exploring additional content may be advisable. It is important to note, however, that these findings are based on studies with participants from the general population or those with elevated symptoms, not individuals formally diagnosed with mental health disorders.

Notes on online therapy platforms

Variability in Therapist Qualifications

The academic background, skills, and experience of therapists (or “providers”) on online psychotherapy platforms vary considerably. While both Online-Therapy.com and Talkspace state that all providers are licensed, licensing standards differ across countries and states, with multiple regulatory bodies applying different criteria.

Some licenses require only a bachelor’s degree, a level of training that, in my opinion, is inadequate for providing psychotherapy.

Inconsistent User Experiences

Review articles on independent websites suggest that the quality of care on these platforms is inconsistent. While some users report positive experiences, others describe unprofessional behaviors, such as therapists texting or driving during sessions, dominating conversations, or offering advice that feels more like chatting with a friend than therapy. These are isolated and potentially biased reports, and it is unclear how common such incidents are. However, one plausible explanation is that lower compensation rates on these platforms may not attract the most experienced or highly trained professionals.

Recommendations When Using an Online Platform

  1. Verify qualifications: Choose a therapist with at least a master’s degree, preferably in psychology. A degree in social work may also be acceptable. Note that online therapy platforms do not always indicate providers’ academic qualifications.
  2. Understand their approach: Make sure the therapist’s clinical orientation – whether cognitive-behavioral, psychodynamic, humanistic, or otherwise –  aligns with your needs and preferences. Take time to learn about the types of interventions they use.
  3. Don’t settle: If your therapist behaves unprofessionally or if you have doubts about their qualifications or methods, seek another provider. A therapeutic relationship should feel professional, safe, and constructive.

Suitability for Different Levels of Need

Online therapy platforms may be suitable for individuals experiencing mild to moderate psychological difficulties. However, they may not be adequate for more complex or severe conditions, such as personality disorders, PTSD, or substance use disorders. In such cases, it is preferable to work with a therapist who has specialized training for your difficulty, often found in specialized clinics.

Final Thoughts

In conclusion, if you are dealing with mild to moderate issues and online platforms offer a more affordable solution where you live, they might be a viable option. However, if costs are comparable or not a concern, I recommend selecting a licensed therapist independently through an official registry in your region (e.g., the American Psychological Association) and ensuring their approach and qualifications match your needs. Many private therapists offer the option of conducting sessions remotely, and support between appointments via messaging is increasingly common.

Research
Brightside’s treatment approach has been evaluated in two studies. The results of these studies were published in peer-reviewd articles, in well-respected scientific journals.

Study 1. Crutchfield, R. M., Zwickey, H., & Miller, A. D. (2022). Effectiveness of a precision-psychiatry platform in treating depression and anxiety: Retrospective cohort study. JMIR Formative Research, 6(7), e36775. https://doi.org/10.2196/36775

– This study looked at over 6,700 people who used Brightside. Most participants reported feeling less anxious or depressed after using the platform, and more than half reached a point where their symptoms were no longer considered clinically significant.

Study 2. Boudreaux, E. D., Crutchfield, R. M., & Miller, A. D. (2023). Evaluation of a technology-enabled mental health platform in primary care: Real-world outcomes of collaborative care for anxiety and depression. Journal of Affective Disorders Reports, 13, 100514. https://doi.org/10.1016/j.jadr.2023.100514

– This study examined how Brightside was used in primary care clinics. It found that people who used the platform showed clear improvements in anxiety and depression symptoms, similar to what you’d expect from traditional in-person care.
